Most access problems don't start with an attacker. They start with a permissions list nobody has looked at in a year. Someone switched teams and kept their old access. A contractor finished a project and their account stayed live. An admin role got handed out for a one-off task and was never taken back. None of it looks dangerous on any given day, and all of it is exactly what an auditor goes looking for.
Access control is where compliance programs get tested hardest, because it's where good intentions and daily practice drift apart the fastest. Role-based access instead of one-off grants
The fastest way to lose control of access is to grant it one person at a time. Every request becomes a judgment call, nobody remembers why a given permission exists, and cleanup is impossible because there's no pattern to follow.
Role-based access flips that. You define roles that map to real jobs, you attach permissions to the role rather than the person, and people inherit access by being assigned a role. When someone's job changes, you change their role, and their access follows. It's easier to reason about, easier to review, and far easier to explain to an auditor who asks why a particular person can reach a particular system.
Roles also make least privilege enforceable. Instead of arguing about individual permissions, you ask a cleaner question: does this role need this access to do its work? If not, it comes out of the role, and everyone assigned to it loses the excess at once.
The joiner, mover, leaver lifecycle
Most access drift traces back to three moments in someone's time with you. Handle these three well and the rest gets much simpler.
Joiners
When someone starts, they should get exactly the access their role calls for, provisioned through a repeatable process rather than a series of favors. A clean start means you're not untangling ad hoc grants months later.
Movers
Internal moves are where least privilege quietly breaks. Someone shifts from support to a different team and keeps their old access on top of their new permissions. Do that a few times across a career and you get people who can reach almost everything. A move should swap access, not stack it.
Leavers
When someone leaves, their access needs to be gone quickly, not eventually. A departed employee or contractor with a live account is one of the clearest findings an auditor can raise, and one of the easiest for an attacker to abuse. Timely deprovisioning is the control here, and "timely" means measured in hours, not weeks.
Access reviews auditors actually believe
Here's the part teams underestimate. A policy that says you review access quarterly proves nothing on its own. Auditors want evidence that the reviews actually happened, on schedule, with someone accountable signing off on what they found.
A real access review means an owner looks at who has access to a system, confirms each person still needs it, and removes what they don't. Then the result gets recorded: who reviewed, when, what changed, and what was left in place. That record is the evidence. Without it, you have a good intention and no way to prove it.
Schedule the review, then keep the receipt
The two things auditors check are that the review ran when it was supposed to and that someone owned the outcome. Put access reviews on a schedule with a named owner, and capture the sign-off as evidence each time, so the proof exists before anyone asks for it.
Privileged access and separation of duties
Not all access is equal. Admin rights, production access, and anything that can move money or change security settings deserve tighter handling than ordinary permissions. Privileged access should go to as few people as possible, be granted for a clear reason, and be reviewed more often than the rest.
Separation of duties works alongside it. No single person should control a sensitive process end to end. The person who requests a change shouldn't be the one who approves it. The person who runs payments shouldn't also reconcile them. Splitting these duties limits both honest mistakes and deliberate abuse, and auditors look specifically for conflicts where one person holds too much of a process.
Multi-factor authentication is table stakes
MFA isn't a nice-to-have anymore. A password alone protects almost nothing, and every framework now treats multi-factor authentication as a baseline expectation rather than an advanced control. If sensitive access isn't behind MFA, that's a finding on its own, before anyone even looks at your reviews.
The practical goal is coverage. MFA on your most important admin account but not the others isn't much better than none. You want it enforced everywhere it matters, and you want to be able to show that it is.
